How does our court order more Case Information Cover Sheets (CICS)?
The case cover sheets (CICS) for Courts of Limited Jurisdiction and for Superior Courts are available in Word format on the Court Forms/Court Forms: Cover Sheets page of the Washington Courts Web site.
The Word document for Superior Courts contains cover sheets for case type 2 and for case types 3 - 6.
Right click Word 97 for the desired document under the Download column and choose Save Target As to save the document to your computer. Click for more detailed download instructions.
